Summary

  • GENERAL SUMMARY
    • The LPN - Primary Care Coordinator works in close cooperation with and under clinical supervision of an RN or physician. Performs a wide variety of patient care activities and accommodative services to support a defined patient population program (Payer Programs, Primary Care Medical Home, NCQA accreditation) and/or service.  Ability to use various technologies, electronic medical records to include patient portal, data base reporting, Excel or Access registries and other vendor’s web based technologies to collect and report various quality or care coordination based data. Reports to Practice Manager/Supervisor. 
  • ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Collaborates and coordinates program with multidisciplinary team to provide quality patient care. 
    • Provides developmentally appropriate psychosocial and clinical care.
    • Participates in formal and informal patient, family and staff education.
    • Demonstrates effective use of time and resources.
    • Performs other duties as assigned.
  • LICENSES AND/OR CERTIFICATIONS
    • Current State license as a Licensed Practical Nurse.
    • CPR Training required and/or must be obtained within 45 days of hire.
  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
    • Professional knowledge of nursing theory and practice.
    • 3 - 5 years clinical experience in a practice setting preferred.
    • Pediatric and program coordination experience preferred.
    • Ability to travel.
    • Must provide record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing.
  • WORKING CONDITIONS
    • Clinical patient care environment with minimal exposure to hazards such as excess noise, dust, or extremes in temperature. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, body fluids and other conditions common to the healthcare environment. Moderate adverse working conditions due to the need to perform certain patient care activities.
  • PHYSICAL REQUIREMENTS

 Monday - Friday 8:30am-5:00pm;  Rotating Evenings 5:00pm,-8:00pm;  Rotating Saturdays 8:30am-Non
