Summary

  • GENERAL SUMMARY
    • The Specialty Program Coordinator LPN works in close cooperation with and under clinical supervision of an RN or physician. Performs a wide variety of patient care activities and accommodative services to support a defined patient population program or service.  Demonstrates excellence in practice characterized by quality patient care and participation in formal and informal patient and staff education. Reports to the Practice Manager and/or Supervisor.
  • ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Collaborates and coordinates program with a multidisciplinary team to provide quality patient care and services. 
    • Provides developmentally appropriate psychosocial and clinical care.
    • Communicates among providers, patient and community resources to enhance the care of the defined population. 
    • Assists in the development and maintenance of policies, procedures, protocols to enhance patient outcomes.
    • Performs other duties as assigned. 
  • LICENSES AND/OR CERTIFICATIONS
    • Current State license as a Licensed Practical Nurse.
  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
    • Professional knowledge of nursing theory and practice. 
    • 3 - 5 years clinical experience in a practice setting required.
    • Pediatric and program coordination experience preferred.
    • CPR Training required and/or must be obtained within 45 days of hire.
  • WORKING CONDITIONS
    • Expected to work in a patient care environment with minimal exposure to environmental hazards such as, but not limited to, excessive noise, dust, or extremes in temperatures. Exposure to communicable diseases and moderately adverse working conditions due to the need to perform certain patient care activities can be expected.
