  • GENERAL SUMMARY
    • The Case Manager is responsible for assisting patients and families within our Mental Health Service Line Outpatient Program. Provides support, resources, and case management services. Collaborates with multidisciplinary teams to reach goals of treatment and provides follow up as necessary. The Case Manager works directly with patients and families to help facilitate and coordinate referrals within all levels of care and needed services.  Works alongside our admin team with scheduling and support our paperwork process. Provides courteous and empathic responses/follow up to patients and families, while determining urgency and level of need within designated timeframes in order to ensure continuity of care. Researches and maintains accurate and up-to-date lists of local mental health resources and their availability in our database. Functions in an on-call capacity as necessary to meet the needs of the department and organization. Reports to the Mental Health Therapy Outpatient Manager. 
  • ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Assesses the biopsychosocial needs, including emotional, social, environmental and financial needs, of the patient and caregiver(s), as well as the limitations or strengths of current available resources.  
    • Accepts and processes new assigned cases through multiple pathways of the outpatient services team.  
    • Collaborates with patient, caregiver(s) and interdisciplinary medical team to develop a service plan that identifies goals, priorities and strategies.  
    • Monitors patient and family’s receipt of services in order to reassess change of needs, as well as to ensure appropriate, sufficient and timely services are being provided. 
    • Documents, as appropriate, in medical record. 
    • Coordinates patient care conference, as appropriate, regarding presenting concerns and case management issues.  
    • Establishes therapeutic relationships with patient and caregiver(s).  
    • Provides counseling and mental health clinical interventions to the patient and/or caregiver, under the supervision of a licensed clinician. 
    • Works with community resources and partners to meet patient service needs. 
    • Participates in regular interdisciplinary meetings, trainings and regular supervision meetings. 
    • Develops appropriate service plan with patients/families for implementing appropriate services and developing effective alliances with patients.  
    • Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the areas identified with the Treatment plan in order to help the patients achieve their goals of treatment.  
    • Performs other duties as assigned.  
  • LICENSES AND/OR CERTIFICATIONS
    • None required.   
  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
    • Graduate of an accredited college or university with a Master’s degree in Social Work, Counseling, or related field. 
    • 1-2 years of related work experience preferred. 
    • Course work in case management, advocacy and family systems preferred. 
    • Preferred experience working in healthcare environments. 
    • Preferred experience with implementing evidence-based treatments with children and families. 
    • Able to convey complex concepts in a diplomatic and concise manner. 
  • WORKING CONDITIONS
    • Normal office environment with little exposure to excessive noise, dust and temperature.
