Summary

  • GENERAL SUMMARY
    • The Interventionist Specialist will provide hospital and community-based services to patients within CHKD Health System who have experienced intentional violent injury through assessment, crisis intervention and counseling. This role reports to the Hospital Violence Intervention Program (HVIP) Program Coordinator and the Director of Mental Health Service Line.

  • ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Collaborates with CHKD Health System staff to identify eligible patients to engage in services and programming offered through participation in the hospital violence intervention program.
    • Collaborates with CHKD program staff, families, and community partners.
    • Discusses and develops a safety plan with youth to avoid retaliatory violence, with support of program coordinator as necessary.
    • Uses a range of communication skills to assist patients, families and staff to address care needs and complex issues.
    • Conducts home visits in the community and provide accompaniment for victims of violent crime within the hospital, criminal justice system, and other systems as needed.
    • Intervenes and supports patients in the Emergency Department and hospital units for patients who have been shot, stabbed, or physically assaulted.
    • Maintains intensive follow up contact with patients, parents/guardians, and community service providers through in-person, telephone, and electronic correspondence.
    • Conduct and assists with group activities, such as group sessions, community events, and focus groups
    • Will assist patients & parents/guardians in applying for Crisis Funds- to ease financial burdens faced by victims and their families.
    • Establishes & maintains positive, professional relationships with coworkers, community agencies, professional organizations, and key stakeholders.
    • Document all patient interactions and collateral contacts in CHKD’s electronic medical records systems according to HIPAA guidelines in accordance with company policy.
  • LICENSES AND/OR CERTIFICATIONS
    • None required.
  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
    • Master’s Degree in Social Work, Counseling or related licensed eligible field.
    • Course work in case management, advocacy, and family systems preferred. 
    • Approximately six to twelve months on-the-job social work experience preferred.
    • Strong knowledge of trauma informed care and practices.
    • Strong public speaking skills.
    • Prior experience working with individuals who have been impacted by violence preferred.
    • Ability to be empathetic, compassionate, and objective when working with patients who have been violently injured. 
    • Knowledge of relevant services, resources, and support available to children and adolescents.
    • Knowledge of positive youth development pathways and ability to provide assessment, counseling, education/support to at-risk youth and families.
    • Demonstrated proactive problem solving, teamwork, innovative thinking, creativity, patience, and flexibility.
    • Demonstrated commitment and ability to support and mentor youth in a positive manner.
    • Hospital and community-based experience preferred.
    • Experience with probation/parole and court services preferred.
    • Knowledge of community resources, human development, and trauma informed practices.
    • Excellent interpersonal and organizational skills.
    • Increased Security Clearance for job role required: FBI finger printing, criminal background check also includes a search of public sex offender and child abuse websites/registries prior to hire and every five years thereafter. Determination of suitability to work with minors will follow the conditions and guidance specified by Office of Justice (OJP) for grant recipients and subrecipients.
  • WORKING CONDITIONS
    • Patient care environment with travel within the community.
